Authorization to Release Patient Information
IMPORTANT: Please be aware that not all personal emails are secured. For this reason, due to HIPAA regulations, please do not include personal health information in your correspondence.
Please use the form below to authorize the release of patient medical records.
- If you are requesting your Pacific Medical Centers records be sent to a provider outside of Pacific Medical Centers, please fax your completed form to (206) 621-4235, or mail your form to: HIM ROI Dept., 1200 12th Ave S, Seattle WA 98144. You can also email us at roi@pacmed.org; please include your name, a contact number, the dates you were seen (as best as you are able) and your date of birth. No fee charged.
- If you are requesting your records from a non-Pacific Medical Centers provider be sent to Pacific Medical Centers, please make your request directly to that provider.
- If you are requesting your records for your own personal use, please follow the directions for #1. There will be a fee for these records.
